Selling of Alcohol License Procedures and State License Forms

APPLICATION INSTRUCTIONS FOR ALCOHOLIC BEVERAGES LICENSES
FORMS REQUIRED BY APPLICANT (Original and five copies of each of the following forms must be filed with the Selectmen's Office). Check with the Selectmen's Office for copies of these forms.
- State ABCC Application Form
- Articles of Incorporation (from Massachusetts Secretary of State)
- Financial Disclosure (one form for each party with a financial interest)
- Financing Form
- Manager experience Form (Form “A”, enclosed)
- Criminal Record Form (one form for each manager and each officer, enclosed)
- Abutters Form - with return of service receipts--required only for new license (List of abutters’ addresses may be obtained from the Assessor's Office, Town Hall 1st floor)
- Dram Shop Liability Insurance Form
- Certificate of Insurance (get from insurance agent)
- Corporate Vote Form
- Floor Plan of store or restaurant
- If the applicant is leasing the premises, a copy of the lease must be included
- Purchase and sale documents, if a transfer
- Massachusetts ABCC Filing Fee of $200 (made payable to the either "Commonwealth of Massachusetts" or "Alcoholic Beverages Control Commission")
- Town of Mattapoisett Application Fee of $50 (made payable to Town of Mattapoisett)
