You will need the following items to apply for a passport:
1. Application for passport, form DS-11
2. Proof of U.S. Citizenship
3. Proof of Identity
4. Two Passport Photographs
5. Applicable Fee
To process passports two checks or money orders are required. For age 16 and over, make one check payable to the US Department of State for $75 and one check payable to the Town of Mattapoisett for $25. If you wish to expedite the passport, add an additional $60 to the $75 for a total of $135 to the US Department of State.
For age 15 and under, make one payable to the US Department of State for $60 and one check payable to the Town of Mattapoisett for $25. If you wish to expedite the passport, add an additional $60 to the $60 for a total of $120.
If you are renewing a passport that was issued less than 15 years ago, there is a form available that you can send in yourself for a new passport. All forms can be downloaded from the Government site. Passport hours are Monday - Friday 8:00AM to 3:30PM. Please call ahead at 508-758-4103 ext 2 to be sure you have all the necessary paperwork beforehand.
There is currently a 6 to 8 week wait. File your application early enough so your passport arrives in time for travel.
